Responsibilities The Wellness Nurse is responsible for conducting the Initial and Subsequent Annual Wellness Visits (AWV) for eligible patients. The Wellness Nurse will assess the physical, emotional, health safety, and community resource needs of the patient and document same according the requirements of the payer.
1. Provides Annual Wellness Visits in the clinics under the direction of the clinic provider and
manager
2. Coordinate with office staff regarding Annual Wellness Visit schedule, verify patient eligibility and schedule AWV appointments when necessary
3. Review, update, and record the patient's past medical/surgical/family/diet/exercise history, other providers/suppliers and vital signs
4. Reconcile the medication list
5. Collect and complete health risk assessment, fall risk assessment, tobacco use assessment, and alcohol assessment
6. Collect and complete a brief cognitive impairment assessment utilizing the Mini-Cog and a PHQ depression screening questionnaire
7. Discuss Advance Care Planning and provide Advanced Directive and Medical Power of Attorney forms for patient if indicated
8. Review, update, and record the patient's annual wellness preventative screenings
9. Maintain current knowledge of policies/procedures and use of office equipment
10. Communicate clearly in person and on the phone, establish/maintain cooperative relationships with patients, families, physicians, and staff
11. Participate in professional development activities and maintain professional licenses and certifications
12. Other duties as assigned
Qualifications Education Level Graduate of an accredited nursing vocational program with 5 years relevant experience
Licensure/Certification Licensed in the state of Texas as an LVN
BLS
Knowledge, Skills, and Abilities •Effective Communication
•Demonstrate excellent interpersonal skills both written and verbal
•Basic computer skills - familiarity with Windows based software programs
Experience •LVN requires 5 years relevant experience
Relevant experience in Homecare, Case Management, Care
Management or Long Term Care Management.
